Generic duloxetine is widely available and dramatically cheaper.

1. Start aerobic exercise — the most effective treatment

Exercise is the most evidence-based treatment for fibromyalgia. Walking, cycling, swimming, water aerobics. Start at 5–10 minutes/day and increase slowly. Most patients who stick with aerobic exercise report significant pain reduction at 12 weeks.

2. Try the three FDA-approved drugs — but adjust expectations

Cymbalta, Lyrica, and Savella are the only FDA-approved fibromyalgia medications. They work for about 1 in 3 patients. Generics are available for all three and are much cheaper. Give each medication 6–8 weeks at full dose before deciding.

3. Fix your sleep — bad sleep makes fibromyalgia worse

Non-restorative sleep amplifies pain sensitivity. Get a sleep study to rule out sleep apnea. Ask about low-dose tricyclic antidepressants at bedtime (amitriptyline, cyclobenzaprine) — cheap generics that improve sleep architecture in fibromyalgia.
